Early during your stay, your health care team will talk to you about your after hospitalization care needs, anticipated discharge date, and how to receive your follow-up care.
If supplies are needed at time of discharge, the discharge nurse will review them with you. If medication is prescribed, it can be picked up at the outpatient pharmacy as you leave the hospital.
If you are on a special diet, a dietitian will counsel you and your family on how to follow your diet at home.
We will work with you to have all discharge arrangements made at least one day prior to your discharge date. If you require help in arranging transportation home or other special needs, the social worker assigned to your ward or clinic can assist you.
Home Care Services
Home care services are available to eligible veterans who need assistance with care in their home after being discharged from the hospital. Arrangements mut be made by your health care team and social worker prior to your discharge.
Nursing Home Facilities
Social workers are available to help you and your family resolve problems affecting your care, discharge planning, VA and communicty resources. If you need counseling or assistance with locating a care facility please see the social worker assigned to your ward or clinic.

If you have lost any personal items during your stay, please contact Textile Care at extension 65700.
Survey of Healthcare Experience of Patients
You may be contacted for feedback about your stay shortly after your discharge. Your comments will help us to continue to meet our goal of providing quality health care to our veteran patients and family. It is important that we have your correct phone number at time of discharge, especially if you will not be going to your own home.
Outpatients may also be contacted for feedback about the services they recieved while visiting the medical center.
